The exact reason is not known, but we do know that a nerve in your face is involved, (trigeminal) creating intense pain around one of your eyes.

How the pain is felt: Commonly on one side, and it stays on the same side during its cycle. When a new headache cycle starts, it might switch to the opposite side, but this is rare.
